Other Services
Looking to obtain services, equipment, or supplies that can improve and/or maintain the participant’s opportunities for full membership in the community, that are not provided through generic resources, the Community Care Program, or through the State Plan? Goods and Services are the answer. Affection Matters LLC can assist the participant with obtaining these goods and services by purchasing them from the participant’s budget.

Respite
Respite services may be provided in the participant’s home or in a hotel. Whether for emergency purposes or to provide relief of those persons who normally provide care for the participant, respite can provide the participant with continued care and/or engagement for a short, previously determined time.

Supported Employment
This service helps the participant obtain and maintain an individual job in competitive or customized employment, or even self-employment, in an integrated work setting in the general workforce for which an individual is compensated at or above the minimum wage, but not less than the customary wage and level of benefits paid by the employer for the same or similar work performed by individuals without disabilities.

Transportation
Need to gain access to services, activities, and resources, up to 150 miles away, but have no way of getting there? Transportation services allow the participant to access the transportation they need through Affection Matters LLC utilizing their DDD budget.

Support Brokerage
While acting as a liaison between the Support Coordinator and Service Providers, the Support Brokerage staff assists the participant (or the participant’s family or representative, as appropriate) in arranging for, directing, and managing services. Serving as the agent of the participant or family, the service is available to assist in identifying immediate and long-term needs, developing options to meet those needs, and accessing identified supports and services. The Support Brokerage staff will communicate with the Support Coordinator to ensure the participant receives continuous care.

Community Based Supports
This service is a one-on-one individualized direct support and assistance for participants, with or without their caregiver present, in or out of the participant’s residence, to achieve and/or maintain increased independence, productivity, enhanced family functioning, and inclusion in the community. Such activities that Affection Matters LLC can provide may include but are not limited to:
- Community Activities (skill building or recreational)
- Daily Living Skills Training (hygiene, cleaning, cooking, etc.)
- Travel Training or Transportation

Community Inclusion Services
Services provided outside of a participant’s home that support and assist participants in educational, enrichment, or recreational activities. These activities are held in a group setting of up to 6 people and are intended to enhance inclusion in the community. Activities may include but are not limited to:
- Early Job Training
- Life Skills Training
- Group Recreational Trips (based on the participants’ desires)

Prevocational Training
This service focuses on providing individuals with learning and work experiences that contribute to their employability in integrated community settings. Through this program, participants can develop general skills and strengths that are valuable for paid employment. Our training covers various aspects, including effective communication, workplace conduct, task-oriented behavior, problem-solving, safety awareness, and mobility skills. The goal is to prepare individuals for competitive employment opportunities where they are compensated fairly.
